This Consumer Health Data Privacy Policy supplements the ELMNTL Privacy Policy (the “Privacy Policy”) and specifically addresses personal data defined as “consumer health data” (“CHD”) as relevant under applicable laws. Undefined capitalized terms herein shall have the meanings provided in our Privacy Policy.

Sources of Consumer Health Data

ELMNTL collects CHD from:

1. Direct interactions with consumers (e.g., intake forms, telehealth consultations, questionnaires).
2. Automated means during use of ELMNTL services or our digital platforms (e.g., website analytics, app interactions).
3. In-store interactions (e.g., IV drips, aesthetics, wellness consultations).

Purposes for Collection of Consumer Health Data

Subject to applicable laws, we collect and use CHD for the following purposes:

ELMNTL may contact you for further information necessary to verify your identity; however, we will never request sensitive personal or financial information, and no employee of ELMNTL will ask for your password.
